For challenge games an application form must be filled in. Brian Teggart must be notified several days before the match with time, date, venue and referee. A sample form is supplied below. Please amend and forward to B.Teggart@Liberty-it.co.uk
The maximum medical payout on any claim is 4500Euro.
Killeavy will payout exactly what comes back in from the scheme, and the payout will be in Euro.
Important Points When Injured
All claims must be lodged within 60 days of the incident.
Claims after 60 days will not be processed by the insurance company. Please inform Brian Teggart in the week the injury has occurred.
If an underage player is injured the team mentor should inform Brian Teggart that week Mobile 07813852581.
Player will be required to complete a claim form.
Keep all receipts (only receipts are accepted by the insurance company not invoices) as only these will be reimbursed by insurance.
If any rehabilitative physiotherapy is required, then this must be referred by a GP.
Willis will not pay out on physio that has not been prescribed by a GP.
